You need to do 2 surgical cases at least 1 case involving 3 or more teeth.
Your cases need to include:
medical history
diagnosis
treatment recommendations
expected outcome
pre op photo
pre op radiograph
photo of initial incision
photo of flapped area
photo of sutured area
3-4 weeks post op photo
3-4 weeks post op radiograph (if any osseous recontouring)
You must also complete the following for both cases.
TX:
1. Informed consent verbal and written
2. Pre op BP
3. Post op BP
4. Anesthesia
5. Incisions
6. Flap
7. Soft tissue removed
8. Root prep
9. Osseous
10. Flap management
11. Sutures
12. Post op instructions
13. Prescriptions
14. EBL and patient condition upon leaving
